San Diego Name and Address Change

If you decide to change your last name to be the same as your husbands you'll do this after you are married and have recieved your marriage certificate. If you decide to keep your name and add his last name to the end of it you still need to go through the process. If you didn't sign the form to recieve it in the mail you'll need to go to one of the county offices, pay the fee and obtain it in person. The good news is that other than purchasing a copy of your marriage certificate name changes in almost every case are usually free of charge, which should come as a welcome relief after just having paid for a wedding.

 

Name Change Checklist

  • Social Security Office
    You must bring a copy of your marriage certificate as well as an identity document showing your old name. Save time, fill out this form before you go.
  • Passport
    You will need to obtain a new passport photo, fill out this form. If you obtained the passport within the year you were married you may change your name for free, otherwise you must renew your old passport, click here form.
  • DMV

  • Mortgage company
  • Property titles or leases
  • Credit card companies
  • Investment accounts, including 401(k)s
  • Car registration and insurance
  • Utility companies (telephone, cell phone, electric, gas, cable, water/sewer, trash, internet)
  • Insurance policies (life insurance as well as health and dental)
  • Doctors and Dentist
  • Voter registration
  • Magazine subscriptions
  • Any memberships, such as your health club or alumni association
